REQUIRED REGISTRATION INFORMATION FAI documents can be faxed/scanned & emailed or dropped off at the Front office or School Nurse for all health-related documents. Proof of Residency (All 3 required) 1. MORTGAGE STATEMENT OR SIGNED LEASE/RENTAL AGREEMENT 2. CURRENT UTILITY BILL (ELECTRIC, PHONE, CABLE) 3. DRIVER'S LICENSE/CAR REGISTRATION Child's birth certificate or passport Legal guardianship/ Custody papers (if applicable) Proof of legal guardianship is also required if you are not the birth parent or if you are divorced, such as: adoption papers, court order clarifying guardianship, guardianship papers from Probate Court, DCF form 603 (must be signed by DCF or legal designee). STATE MANDATED PHYSICAL & IMMUNIZATIONS HEALTH ASSESSMENTS must be completed prior to entry into school. State Mandated Kindergarten Physicals must be completed between January 1st and Dec. 31st of the calendar year the child starts school.
